Former victims’ commissioner says Southport failures must be met with accountability

A former victims’ commissioner has said the families affected by the Southport murders cannot be certain that the officials who made “catastrophic” failures before the killings are not still making the same mistakes.

Vera Baird KC said those responsible for failing to properly monitor the killer, Axel Rudakubana, should be held “personally accountable”. She argued that authorities must not simply “shrug it off” with an apology.

Her comments come amid renewed scrutiny of the errors that took place before the murder of three girls in Southport. Baird said disciplinary action is essential if public bodies are to ensure that people are properly held to account for serious failings.

The warning reflects wider concern that lessons from the case may not yet have been fully learned. Baird’s remarks suggest that, without clear consequences, the same mistakes could be repeated by those working in systems meant to identify and manage risk.

According to Baird, accountability should not stop at expressions of regret. She said those who failed to properly monitor Rudakubana should face personal responsibility for what happened, rather than allowing the matter to end with an apology alone.

The Southport murders have prompted intense questions about what happened before the attack and whether agencies involved acted appropriately. Baird’s intervention adds pressure on authorities to show that they are taking the failings seriously and addressing them in a way that goes beyond words.

Her position is that discipline matters not only as a response to past errors, but also as a safeguard for the future. In her view, public confidence depends on whether officials who fall short are genuinely held responsible when their mistakes have severe consequences.
